Roles & Responsibilities

• Provide compassionate direct patient care and assist with activities of daily living
• Monitor and record vital signs and report changes to the nursing team
• Support safe patient mobility, transfers, and positioning
• Maintain accurate, timely documentation of care and observations
• Follow infection control practices and facility policies consistently
• Communicate clearly and kindly with patients, families, and staff to promote coordination of care
• Contribute to a positive unit culture by supporting teammates and assisting with unit needs

Qualifications

• Valid LNA (Licensed Nursing Assistant) or T-LNA credential (VT/NH licensing aligned with job region)
• Ability to work night shifts and every-other-weekend rotations
• Reliable transportation to and from the facility (explicit requirement)
• Current basic competency in patient care; recent clinical experience preferred
• CPR/BLS certification preferred or willingness to obtain

Education

• High school diploma or equivalent
• Completion of an approved LNA training program and current state LNA certification

Knowledge & Skills

• Comfort with activities of daily living assistance and patient hygiene
• Basic vital sign measurement and observation skills
• Strong communication and teamwork skills
• Good time management and the ability to prioritize in a busy unit
• Familiarity with infection prevention and safety protocols
• Dependable documentation and charting habits
