Roles & Responsibilities

• Perform thorough patient evaluations and develop individualized treatment plans that restore function and independence
• Deliver hands-on therapy and evidence-based interventions to improve mobility, strength, and balance
• Collaborate with nursing, case managers, and family members to coordinate care and optimize outcomes
• Document patient progress clearly and timely in the facility’s records to ensure continuity of care
• Provide patient and caregiver education on home exercise programs, safety, and fall prevention
• Participate in discharge planning and communicate recommendations to support safe transitions
• Maintain compliance with facility policies, professional standards, and state regulations

Qualifications

• Current license to practice as a Physical Therapist in Massachusetts (or ability to obtain prior to start)
• Basic Life Support (BLS) certification preferred or willingness to obtain
• Recent experience in skilled nursing, rehabilitation, or post-acute care settings preferred
• Strong clinical reasoning, communication skills, and a patient-focused approach
• Ability to pass background check and required pre-employment screenings

Education

• Doctor of Physical Therapy (DPT) or Bachelor's/Master’s degree in Physical Therapy as required by state licensure

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise, manual therapy, and gait training
• Experience with mobility aids, transfers, and fall prevention strategies
• Comfortable developing and implementing individualized care plans
• Proficient documentation and charting practices
• Strong interdisciplinary teamwork and patient/family education skills
• Ability to adapt evidence-based interventions to a post-acute/skilled nursing population
