Roles & Responsibilities
- Perform thorough patient evaluations and develop individualized treatment plans that restore function and independence
- Deliver hands-on therapy and evidence-based interventions to improve mobility, strength, and balance
- Collaborate with nursing, case managers, and family members to coordinate care and optimize outcomes
- Document patient progress clearly and timely in the facility’s records to ensure continuity of care
- Provide patient and caregiver education on home exercise programs, safety, and fall prevention
- Participate in discharge planning and communicate recommendations to support safe transitions
- Maintain compliance with facility policies, professional standards, and state regulations
Qualifications
- Current license to practice as a Physical Therapist in Massachusetts (or ability to obtain prior to start)
- Basic Life Support (BLS) certification preferred or willingness to obtain
- Recent experience in skilled nursing, rehabilitation, or post-acute care settings preferred
- Strong clinical reasoning, communication skills, and a patient-focused approach
- Ability to pass background check and required pre-employment screenings
Education
- Doctor of Physical Therapy (DPT) or Bachelor's/Master’s degree in Physical Therapy as required by state licensure
Knowledge & Skills
- Skilled in patient assessment, therapeutic exercise, manual therapy, and gait training
- Experience with mobility aids, transfers, and fall prevention strategies
- Comfortable developing and implementing individualized care plans
- Proficient documentation and charting practices
- Strong interdisciplinary teamwork and patient/family education skills
- Ability to adapt evidence-based interventions to a post-acute/skilled nursing population
