Roles & Responsibilities
- Provide compassionate, hands-on nursing care to patients according to plan
- Administer medications and treatments safely and accurately
- Perform timely patient assessments and communicate changes to the care team
- Document patient status and interventions clearly in the medical record
- Collaborate with nurses, providers and support staff to deliver coordinated care
- Support patient and family education with patience and empathy
- Uphold facility policies, patient safety standards and infection control practices
Qualifications
- Active, unencumbered LPN license (state licensure required)
- Ability to work rotating 12-hour day shifts as described (local, on-site)
- Previous LPN experience in a clinical or long-term care setting preferred
- Strong communication skills and a team-oriented mindset
- Current basic life support (BLS) certification if required by facility (preferred)
Education
- Graduate of an accredited LPN/LVN program or equivalent
Knowledge & Skills
- Solid clinical assessment and bedside care skills
- Medication administration and documentation expertise
- Comfortable with electronic health records and accurate charting
- Time management and prioritization during busy 12-hour shifts
- Compassionate patient education and family communication
