Roles & Responsibilities
- Evaluate patients with speech, language, cognitive, and swallowing disorders to build a clear clinical picture
- Develop and implement individualized treatment plans that target meaningful functional gains
- Provide skilled therapy services focused on communication, cognition, and dysphagia management
- Document patient progress and treatment outcomes accurately and professionally
- Collaborate with interdisciplinary healthcare teams to support optimal patient outcomes
- Educate patients and caregivers on therapy goals, home strategies, and safety to maximize carryover
Qualifications
- Active California Speech-Language Pathologist license (required)
- Experience in a Skilled Nursing Facility or rehabilitation setting preferred
- Strong clinical judgment with excellent communication skills
- Ability to work independently while contributing positively to a team
- Comfort with skilled documentation and outcome-driven care
Knowledge & Skills
- Assessment and treatment of communication disorders
- Dysphagia evaluation and management (swallowing disorders)
- Cognitive-communication and language disorder interventions
- Skilled documentation and tracking of treatment outcomes
- Interdisciplinary collaboration and patient/caregiver education
- Familiarity with rehab-focused SNF workflows and regulatory expectations
