Roles & Responsibilities
- Evaluate patients and craft individualized physical therapy treatment plans tailored to functional goals
- Deliver skilled therapy focused on mobility, strength, balance, and restoring independence
- Monitor and document patient progress accurately and in a timely manner to guide care
- Collaborate with interdisciplinary teams to support optimal patient outcomes and care transitions
- Educate patients and caregivers on therapy goals and practical home exercise programs
Qualifications
- Active California Physical Therapist license (required)
- Experience in a Skilled Nursing Facility or rehabilitation setting preferred
- Strong clinical judgment and excellent communication skills
- Ability to work independently while contributing positively to a collaborative team
Knowledge & Skills
- Solid command of physical therapy principles and patient-centered care
- Skilled in mobility, balance, strength training, and functional task training
- Clear and timely documentation practices
- Comfortable working with interdisciplinary teams and educating patients/caregivers
- Professional communication and clinical decision-making
