Shift Type: Rotating 12-hour shifts (cycle scheduling): 6:00 AM - 2:30 PM; 2:00 PM - 11:00 PM; 10:00 PM - 6:30 AM, cycle scheduling. Every other weekend and every other holiday required. (Day and night coverage; rotating schedule)
Hours per Week: 36
Candidate Pay Rate: $46 per hour

Roles & Responsibilities
- Provide resident-centered nursing care as an LPN/LVN on an LTC/SNF unit
- Administer medications and document medication administration accurately
- Chart resident care and clinical observations in PointClickCare (PCC)
- Monitor residents for changes in condition and report concerns to the charge nurse or RN
- Support ADLs and collaborate with CNAs and interdisciplinary team members
- Carry out care plans for residents with CHF, COPD, Alzheimer’s, Parkinson’s, and Diabetes
- Participate in unit orientation and mentor newer staff during their onboarding
- Help maintain a safe, respectful, and home-like environment for residents
Qualifications
- Current LPN/LVN license (must be valid for practice in Minnesota)
- BLS certification required
- Minimum 1 year of recent nursing experience (experience in long-term care / SNF preferred)
- Experience or comfort administering medications and minimal IV meds
- Proficiency with PointClickCare charting preferred/required
- Ability to work rotating 12-hour shifts including every other weekend and holidays
- Eligibility to work per facility hiring policies (radius/no recent affiliation restrictions apply)
Education
- LPN/LVN licensure required (vocational nursing diploma or equivalent program completion). BSN not required.
Knowledge & Skills
- Medication administration and safe documentation practices
- Long-term care resident assessment and chronic disease management (CHF, COPD, Diabetes)
- Dementia care approaches for residents with Alzheimer’s and related disorders
- Strong teamwork, communication, and time-management skills
- Familiarity with PointClickCare (PCC) electronic charting
- Ability to follow care plans and escalate clinical concerns appropriately
