Roles & Responsibilities

• Perform skilled patient assessments in the home setting and develop individualized treatment plans
• Provide hands-on therapy and instruct patients and caregivers in safe mobility and home exercise programs
• Manage a daily caseload of approximately 5-7 patients with clear, timely documentation in HomeCare/HomeBase EMR
• Coordinate care with hospice teams, nurses, and other interdisciplinary partners to optimize outcomes
• Travel within the assigned radius to visit patients, prioritize visits, and maintain efficient route planning
• Deliver compassionate, patient-focused care for common diagnoses such as post-surgical recovery and fall prevention
• Participate in required orientation and ongoing training to maintain high standards of home health practice

Qualifications

• Current North Dakota Physical Therapy license (required)
• Basic Life Support (BLS) certification (required)
• Minimum 1 year of physical therapy experience, home health experience strongly preferred
• Valid driver’s license and current auto insurance to cover travel within the service radius
• Comfortable and proficient with HomeCare/HomeBase EMR or similar electronic charting systems
• Able to work independently in the field without a PT manager on shift and handle clinical decision-making

Knowledge & Skills

• Strong patient assessment and treatment planning skills tailored to home health and hospice populations
• Skilled in mobility training, fall risk assessment, and post-operative rehabilitation
• Proficient documentation practices and experience with HomeCare/HomeBase or comparable EMR
• Excellent time management, route planning, and caseload organization for efficient home visits
• Clear communication and caregiver education skills to support family-centered home care
• Ability to adapt treatment approaches to varied home environments and equipment availability
