Roles & Responsibilities

• Deliver hands-on physical therapy interventions and assist patients in their homes with dignity and compassion
• Assess patient mobility, functional status, and progress toward therapy goals
• Document all patient care thoroughly and accurately using Cerner and complete OASIS forms as required
• Collaborate with PTs, OTs, SLPs and rehab aides to develop and adjust individualized care plans
• Educate patients and caregivers on exercises, safety strategies, and home modifications
• Coordinate visits within an assigned geographic region to ensure continuity of care
• Participate in orientation and team meetings; support a culture of continuous improvement
• Adhere to professional dress and conduct standards (business attire with white lab coat; no jeans)

Qualifications

• Current PTA credential or completion of an accredited Physical Therapy Assistant program (per state requirements)
• BLS (Basic Life Support) certification required
• Minimum 2 years of clinical experience (job details indicate 2 years); at least 1 year working with PT/OT/SLP populations preferred
• Required experience with OASIS documentation (government-mandated forms)
• Proficiency with Cerner EMR for patient charting
• Comfort providing care in home health settings and ability to work within an assigned geographic region

Education

• Completion of an accredited Physical Therapy Assistant program (Associate degree or equivalent)

Knowledge & Skills

• Strong knowledge of orthopedic, neurological (stroke), joint and spinal injury rehabilitation
• Skilled in patient assessment, therapeutic exercise, gait training, and safety education
• Excellent documentation and clinical reasoning skills, especially with OASIS
• Good communication and collaboration skills to work with interdisciplinary teams
• Time management and route planning for efficient home visits
• Ability to adapt treatment plans to varied home environments and patient needs
