Shift Type: Monday-Friday, 8:00 AM - 5:00 PM (Day); every 3rd weekend required; occasional major-holiday coverage as needed
Hours per Week: 40
Candidate Pay Rate: $49 per hour

Roles & Responsibilities
- Deliver hands-on physical therapy interventions and assist patients in their homes with dignity and compassion
- Assess patient mobility, functional status, and progress toward therapy goals
- Document all patient care thoroughly and accurately using Cerner and complete OASIS forms as required
- Collaborate with PTs, OTs, SLPs and rehab aides to develop and adjust individualized care plans
- Educate patients and caregivers on exercises, safety strategies, and home modifications
- Coordinate visits within an assigned geographic region to ensure continuity of care
- Participate in orientation and team meetings; support a culture of continuous improvement
- Adhere to professional dress and conduct standards (business attire with white lab coat; no jeans)
Qualifications
- Current PTA credential or completion of an accredited Physical Therapy Assistant program (per state requirements)
- BLS (Basic Life Support) certification required
- Minimum 2 years of clinical experience (job details indicate 2 years); at least 1 year working with PT/OT/SLP populations preferred
- Required experience with OASIS documentation (government-mandated forms)
- Proficiency with Cerner EMR for patient charting
- Comfort providing care in home health settings and ability to work within an assigned geographic region
Education
- Completion of an accredited Physical Therapy Assistant program (Associate degree or equivalent)
Knowledge & Skills
- Strong knowledge of orthopedic, neurological (stroke), joint and spinal injury rehabilitation
- Skilled in patient assessment, therapeutic exercise, gait training, and safety education
- Excellent documentation and clinical reasoning skills, especially with OASIS
- Good communication and collaboration skills to work with interdisciplinary teams
- Time management and route planning for efficient home visits
- Ability to adapt treatment plans to varied home environments and patient needs
