Roles & Responsibilities

• Provide compassionate hands-on care to residents, assisting with activities of daily living
• Monitor and report changes in patient condition to nursing staff promptly
• Support safe patient transfers and mobility with attention to dignity and comfort
• Document care accurately and timely in the facility charting system
• Follow infection control and safety protocols to protect residents and team members
• Collaborate with nurses, therapists, and interdisciplinary staff to deliver person-centered care
• Assist with mealtime, bathing, grooming, and toileting while encouraging resident independence

Qualifications

• Active Licensed Nursing Assistant (LNA) credential — NH required; VT acceptable if floating to VT
• Completion of an approved LNA training program
• Current CPR/BLS certification (or willingness to obtain prior to start)
• Comfortable working the 3:00 PM - 11:00 PM shift and supporting weekend coverage
• Strong communication skills and a patient-centered approach
• Previous long-term care or skilled nursing experience is a plus

Education

• High school diploma or equivalent
• Completion of a state-approved LNA program

Knowledge & Skills

• Skilled in assisting with ADLs and resident mobility
• Ability to accurately record vitals and observe changes in condition
• Familiarity with basic charting and documentation practices
• Knowledge of infection control, safety, and resident rights
• Team-oriented mindset with good interpersonal and communication skills
• Time management and ability to prioritize resident needs during busy shifts
