Roles & Responsibilities
- Provide compassionate, professional nursing care to clients of all ages in the home setting
- Perform admissions, thorough evaluations, and develop individualized care plans
- Coordinate care with a team of homecare professionals and paraprofessionals to ensure continuity
- Educate patients and families on care plans, medications, and safety measures
- Document clinical findings, interventions, and outcomes accurately and timely
- Advocate for patients’ needs and collaborate with community resources to support care goals
- Monitor and report changes in patient condition, using clinical judgment to escalate when needed
Qualifications
- Current Vermont RN license (required)
- Active Registered Nurse with at least 1 year of prior clinical experience
- Comfortable providing care across the lifespan (experience with all ages preferred)
- Team-oriented mindset with strong communication and collaboration skills
- Willingness to work occasional weekends as needed
- Experience with hospice care is a plus

- Skilled patient care and clinical assessment
- Ability to coordinate and collaborate with paraprofessionals and interdisciplinary teams
