Roles & Responsibilities
- Assess patients' physical abilities and limitations to create individualized treatment plans
- Deliver evidence-based therapeutic interventions to restore mobility and function
- Collaborate with nurses, physicians and therapists to coordinate high-quality care
- Document progress clearly and timely in the medical record and communicate goals
- Educate patients and families on safe mobility, home exercise programs and fall prevention
- Supervise therapy assistants and aides while maintaining clinical accountability
- Participate in discharge planning to ensure smooth transitions and community safety
- Contribute to a culture of safety, compliance and continuous improvement
Qualifications
- Licensed Physical Therapist in Massachusetts (or eligible for state licensure)
- Doctor of Physical Therapy (DPT) preferred; related degrees considered
- Basic Life Support (BLS) certification required or obtain upon start
- Experience in skilled nursing, inpatient rehab or geriatrics preferred
- Strong clinical reasoning, documentation skills and effective communication
- Ability to work independently and as part of a multidisciplinary team
Education
- Doctor of Physical Therapy (DPT) preferred
- Bachelor of Science in Physical Therapy or equivalent considered
Knowledge & Skills
- Skilled in assessment and development of therapeutic treatment plans
- Proficient with manual therapy, therapeutic exercise and mobility training
- Knowledge of evidence-based modalities for musculoskeletal and neurological conditions
- Familiarity with EMR documentation and regulatory compliance in post-acute settings
- Strong patient education and caregiver training abilities
- Good time management, triage and caseload prioritization skills
