Roles & Responsibilities

• Conduct thorough OASIS assessments and document findings accurately in the EMR (HealthWyse)
• Provide skilled physical therapy care to home health and hospice patients in their residences
• Plan, implement, and adapt individualized treatment plans based on patient goals and changing needs
• Travel between patient homes safely and efficiently, managing time and caseload
• Coordinate care with interdisciplinary team members and communicate patient updates clearly
• Educate patients and caregivers to promote safety, independence, and functional progress
• Maintain timely, compliant documentation and support quality and regulatory requirements
• Adjust schedules and priorities as patient needs evolve, including holiday coverage when needed

Qualifications

• Experience performing OASIS assessments (required)
• Proven home health and hospice physical therapy experience (required)
• Proficiency with EMR documentation; experience with HealthWyse is strongly preferred
• Current, active Physical Therapist license as required by state regulations
• Valid driver’s license and reliable transportation for travel between patient homes
• Strong time-management skills and ability to adapt to changing schedules
• Willingness to complete facility background check and comply with onboarding requirements

Education

• Degree in Physical Therapy (Doctor of Physical Therapy preferred or equivalent)

Knowledge & Skills

• In-depth knowledge of OASIS assessment processes and home health regulatory requirements
• Skilled in home-based therapeutic interventions and functional mobility training
• Competent documentation and clinical reasoning in an EMR environment (HealthWyse)
• Excellent communication and patient/caregiver education skills
• Ability to manage caseloads, prioritize visits, and navigate community resources
• Comfort working independently in patients’ homes while maintaining clinical standards
