Roles & Responsibilities

• Deliver skilled physical therapy assessments and treatments in patients' homes with compassion and clinical excellence
• Develop and implement individualized treatment plans focused on functional outcomes and safe home mobility
• Educate patients and caregivers on strategies for independence, fall prevention, and home exercise programs
• Collaborate with nurses, occupational therapists, social workers, and case managers to coordinate holistic care
• Document clinical findings, progress, and discharge plans accurately and timely in the EMR
• Perform home safety evaluations and recommend durable medical equipment or environmental modifications
• Advocate for patient needs and contribute to quality improvement initiatives within the home health team

Qualifications

• Minimum 2 years of physical therapy experience (home health experience strongly preferred)
• Valid Physical Therapist (PT) license in the state of practice or eligibility to obtain licensure
• Current BLS/CPR certification
• Comfortable driving to patient homes and managing a mobile caseload (valid driver’s license and reliable transportation)
• Strong clinical reasoning, documentation skills, and ability to work autonomously

Education

• Degree in Physical Therapy (DPT preferred) or equivalent professional PT degree

Knowledge & Skills

• Skilled in functional mobility assessment and therapeutic exercise prescription
• Knowledge of home safety evaluation and fall prevention strategies
• Familiarity with home health regulatory and documentation requirements
• Excellent communication, patient education, and caregiver coaching skills
• Time management and route planning to optimize patient visits
