Roles & Responsibilities

• Provide skilled physical therapy evaluations and treatments in the home setting that improve patient function and independence
• Develop individualized treatment plans and measurable goals based on thorough assessments
• Document care accurately and promptly in EPIC (electronic charting is part of the role)
• Coordinate with interdisciplinary team members and family caregivers to ensure continuity of care
• Coach patients and caregivers on safe mobility, fall prevention, and home exercise programs
• Maintain required competencies and complete Trinity specialty checklists and testing as directed

Qualifications

• BLS certification required
• License/certificate verification (state licensure and relevant credentials will be verified)
• Comfortable using EPIC for documentation and care coordination
• Completion of Trinity Skills Checklist and Trinity Specialty Specific Competency Testing as part of onboarding
• Ability to pass Trinity 10 Panel Drug Screen, respirator evaluation, and fit test
• Up-to-date immunizations and health screenings per employer requirements (COVID-19, MMR, Varicella, Hep B, Influenza, Tdap, 2-step PPD or IGRA)

Knowledge & Skills

• Proven skills in patient assessment, therapeutic exercise, and functional mobility training
• Strong clinical reasoning and ability to set measurable treatment goals
• Effective verbal and written communication with patients, families, and care teams
• Experience with home safety evaluations and adaptive equipment recommendations
• Familiarity with electronic medical records (EPIC) and timely documentation practices
• Ability to complete required competency testing and adhere to organizational checklists
