Roles & Responsibilities
- Evaluate and treat residents in skilled nursing and post-acute settings to maximize functional independence
- Develop and implement individualized treatment plans that help residents reach their highest potential
- Collaborate with an interdisciplinary team to coordinate care and improve outcomes
- Document treatments and progress accurately and timely in compliance with facility and regulatory standards
- Participate in care conferences and contribute to quality improvement initiatives
- Educate residents and families on safe mobility, adaptive strategies, and home programs
- Advocate for resident-centered care and support a positive therapeutic environment
Qualifications
- Active California Physical Therapist license (required)
- Strong clinical reasoning and hands-on patient evaluation skills
- Skilled nursing or post-acute care experience preferred
- Excellent communication and interdisciplinary collaboration abilities
- Dependable, flexible, and team-oriented approach to care
- Commitment to delivering high-quality, compassionate therapy

- Skilled nursing/post-acute care practices and regulatory documentation
