Roles & Responsibilities
- Conduct screenings, assessments, diagnoses and treatment planning for patients from birth through elder
- Provide brief therapy and continuing care to strengthen families and support recovery
- Lead and facilitate discharge planning and coordinate ALF or transitional placements
- Collaborate with hospital teams and community partners to ensure seamless transitions of care
- Advocate for clients and assist with complex matters such as Power of Attorney when appropriate
- Maintain accurate, timely documentation to meet state, federal, accreditation and billing standards
- Participate in multidisciplinary team meetings and contribute to quality improvement efforts
- Support staff training and help integrate new practices into client services
Qualifications
- Master's degree in Social Work, Psychology, Counseling or a related discipline from an accredited institution
- Licensed (or in the process of becoming licensed) as an LCSW, LPC, or LMFT
- Valid State of Alaska driver’s license or ability to obtain within 6 months of hire
- Must be at least 21 years of age
- Clinical Competency to be completed within 3 months of hire and maintained every three years
- Demonstrated experience in discharge planning, care coordination, case management and clinical documentation
Education
- Master's Degree in Social Work, Psychology, Counseling or related discipline (accredited institution)
Knowledge & Skills
- Discharge planning and care coordination across the continuum
- Clinical assessment, screening, diagnosis and treatment planning
- Brief therapy approaches and crisis intervention techniques
- Chemical dependency and substance use assessment knowledge
- Familiarity with tribal health principles and cultural wellness
- Strong documentation, compliance and billing documentation practices
- Ability to facilitate multidisciplinary team meetings and patient care conferences
- Client advocacy, skill-building and resource navigation
