Roles & Responsibilities
- Evaluate patients in their homes to create meaningful, individualized care plans
- Deliver skilled physical therapy interventions that improve mobility, safety, and independence
- Document assessments and progress thoroughly using HCHB so the whole team stays informed
- Coordinate with nurses, caregivers, and families to ensure seamless continuity of care
- Educate patients and caregivers on home exercise programs and fall prevention strategies
- Manage a daily caseload of patient visits while prioritizing quality and safety
- Communicate clearly with the clinical team about changes in patient status and care needs
Qualifications
- Licensed Physical Therapist (state license required) — active and in good standing
- Minimum 2 years of clinical experience (home health experience preferred and highlighted)
- Home health and hospice experience strongly preferred for success in this role
- Familiarity with HCHB documentation system or strong aptitude to learn it quickly
- Valid driver's license and a reliable vehicle for patient visits across the service area
- Current CPR/BLS certification as required by employer or state
Education
- Doctor of Physical Therapy (DPT) or Master's in Physical Therapy (as applicable/licensed)
Knowledge & Skills
- Expertise in home health evaluation and treatment planning
- Strong documentation skills with HCHB or similar electronic charting systems
- Excellent patient education and coaching abilities
- Time management and route planning for efficient patient visit days
- Ability to work independently while collaborating with an interdisciplinary team
- Compassionate bedside manner and strong communication skills with patients and families
