Roles & Responsibilities

• Perform thorough physical therapy evaluations and develop individualized plans of care
• Provide hands-on treatment in the rehab gym, patient rooms, hallways, and outdoor therapy spaces
• Collaborate with the interdisciplinary rehab team during rounds and care conferences
• Document patient progress accurately using Cerner and NetHealth and communicate needed equipment and discharge needs
• Float between acute/swing, nursing home, and outpatient services to support continuity of care
• Educate patients and families to maximize functional outcomes and safe discharges

Qualifications

• Current NC Physical Therapy license (required)
• Valid CPR certification (required)
• Minimum 2 years of clinical experience (hospital and/or nursing home experience required)
• Prefer clinicians with at least 3 years of experience in a medical setting
• Comfortable documenting in Cerner and NetHealth
• Ability to float across acute, swing, nursing home, and outpatient settings

Knowledge & Skills

• Skilled in physical therapy evaluations and establishing effective plans of care
• Experience with rehab services and typical rehab equipment
• Proficient documentation practices (Cerner and NetHealth)
• Strong communication and teamwork with rehab colleagues and nursing staff
• Ability to manage 1:1 patient ratios and prioritize care needs
• Adaptability to treat in varied settings including gym, rooms, hallways, and outdoor areas
