Roles & Responsibilities

• Perform thorough evaluations to assess residents' physical abilities and develop individualized treatment plans
• Deliver physical therapy interventions to improve strength, mobility, and functional independence
• Educate residents and families on therapeutic exercises, safe movement, and home modifications
• Collaborate with interdisciplinary team members—OTs, nurses, physicians—to coordinate care and optimize outcomes
• Monitor progress and adjust treatment plans to ensure the best possible results
• Maintain accurate, timely clinical documentation in accordance with facility policies and regulatory requirements

Qualifications

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
• Valid Physical Therapy license in the state of California (or ability to obtain prior to start)
• Experience in a skilled nursing facility or similar post-acute setting preferred
• Strong clinical knowledge of therapeutic modalities, adaptive equipment, and treatment planning
• Excellent communication, interpersonal, and problem-solving abilities
• Ability to work independently while contributing positively to a team-focused environment
• Compassionate, patient-centered approach to care

Education

• Bachelor's, Master's, or Doctoral degree in Physical Therapy (from an accredited program)

Knowledge & Skills

• Patient assessment and development of personalized treatment plans
• Therapeutic exercise and therapeutic modalities
• Use of adaptive equipment and rehabilitation techniques
• Clinical documentation and progress note best practices
• Patient and family education for safe mobility and home exercise programs
