Roles & Responsibilities
- Perform thorough evaluations to assess residents' physical abilities and develop individualized treatment plans
- Deliver physical therapy interventions to improve strength, mobility, and functional independence
- Educate residents and families on therapeutic exercises, safe movement, and home modifications
- Collaborate with interdisciplinary team members—OTs, nurses, physicians—to coordinate care and optimize outcomes
- Monitor progress and adjust treatment plans to ensure the best possible results
- Maintain accurate, timely clinical documentation in accordance with facility policies and regulatory requirements
Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
- Valid Physical Therapy license in the state of California (or ability to obtain prior to start)
- Experience in a skilled nursing facility or similar post-acute setting preferred
- Strong clinical knowledge of therapeutic modalities, adaptive equipment, and treatment planning
- Excellent communication, interpersonal, and problem-solving abilities
- Ability to work independently while contributing positively to a team-focused environment
- Compassionate, patient-centered approach to care
Education
- Bachelor's, Master's, or Doctoral degree in Physical Therapy (from an accredited program)
Knowledge & Skills
- Patient assessment and development of personalized treatment plans
- Therapeutic exercise and therapeutic modalities
- Use of adaptive equipment and rehabilitation techniques
- Clinical documentation and progress note best practices
- Patient and family education for safe mobility and home exercise programs
- Interdisciplinary collaboration and care coordination
- Strong interpersonal skills and professional communication
