Roles & Responsibilities

• Treat patients as directed by the supervising Physical Therapist, delivering hands-on care with compassion and skill
• Record daily treatment notes and weekly progress notes in compliance with PT Board requirements
• Assist in maintaining a safe, organized, and well-stocked treatment area
• Collaborate in Patient Care and Rehabilitation Conferences to drive meaningful progress for residents
• Train and support restorative aides and nursing staff to reinforce continuity of care
• Instruct patients and families on maintenance programs and caregiver strategies for successful discharge
• Report equipment concerns promptly and assist with basic maintenance to ensure safe therapy delivery
• Communicate patient progress, problems, and plans clearly with supervisors and the interdisciplinary healthcare team

Qualifications

• Licensed Physical Therapist Assistant in good standing with the state of California
• Proficient in clinical documentation and electronic charting systems such as Casamba and PointClickCare
• Comfortable providing direct patient care in a long-term care / rehabilitation setting
• Experience or willingness to train restorative aides and participate in in-service training
• Strong communication skills and the ability to work positively with patients, families, nursing staff, and colleagues
• Must meet continuing education requirements per state practice and maintain license throughout assignment

Knowledge & Skills

• Hands-on physical therapy assistance and patient-centered care
• Clinical documentation best practices and weekly progress note preparation
• Familiarity with Casamba and PointClickCare electronic health record systems
• Caregiver training techniques and discharge planning education
• Basic equipment maintenance and treatment area upkeep
• Effective interdisciplinary communication and teamwork
• General computer skills for data entry and clinical recordkeeping
