Roles & Responsibilities

• Screen, evaluate and re-evaluate inpatients and outpatients to develop individualized therapy plans
• Deliver evidence-based physical therapy services across the lifespan — from pediatric to adult patients
• Coordinate care and communicate effectively with physicians, nurses and the broader healthcare team
• Provide clear, timely clinical documentation and participate in quality assessment activities
• Collaborate with multi-disciplinary teams to optimize patient outcomes and discharge planning
• Educate patients and families about treatment plans, home programs and safe functional progressions

Qualifications

• BS in Physical Therapy from an accredited physical therapy program (required)
• Current New Hampshire Physical Therapy license (required)
• BLS (AHA) certification (required)
• At least 1 year of clinical PT experience preferred but new graduates from accredited programs are encouraged to apply
• Excellent interpersonal skills and the ability to function as a valued member of a multi-disciplinary team
• Experience with EPIC or other electronic medical records is a plus

Education

• BS in Physical Therapy (from an accredited physical therapy program)

Knowledge & Skills

• Patient assessment and treatment planning for inpatient and outpatient populations
• Clinical documentation and quality assessment practices
• Care coordination and effective interdisciplinary communication
• Therapy services across pediatric to adult age ranges
• Strong interpersonal skills and team collaboration
• Familiarity with EPIC EMR workflows
