Roles & Responsibilities

• Provide skilled physical therapy evaluations and treatments in patients' homes, helping them regain independence
• Design and implement individualized treatment plans based on functional goals and clinical findings
• Perform thorough assessments of mobility, balance, strength, and safety in the home environment
• Collaborate with nurses, case managers, and caregivers to coordinate holistic care
• Document timely, accurate patient encounters and progress notes (Epic preferred)
• Educate patients and families on home exercise programs, fall prevention, and safe mobility strategies
• Maintain competency by completing Trinity specialty-specific testing and skills checklists

Qualifications

• Valid Physical Therapy license for Iowa (must pass license verification)
• BLS certification required
• Home health experience preferred
• Completion of Trinity specialty competency testing and skills checklist required
• Ability to pass health screenings: immunizations, drug screen, TB testing, and COVID-19 vaccine requirements
• Comfortable using electronic health records; Epic experience preferred but not required

Education

• Graduate of an accredited Physical Therapy program (DPT preferred)

Knowledge & Skills

• Strong assessment and clinical reasoning skills for home health populations
• Experience with mobility, transfers, fall risk assessment, and ADL training
• Proficient documentation and time-management skills
• Effective communication with patients, families, and interdisciplinary teams
• Familiarity with infection control and safety in the home environment
• Comfortable learning and using Epic or similar EHR systems
