Roles & Responsibilities

• Provide compassionate direct care to residents following individualized care plans
• Administer medications and treatments safely and accurately during the shift
• Monitor resident conditions, document changes, and communicate findings to the care team
• Support ADLs (bathing, dressing, mobility) while encouraging resident independence
• Collaborate with RNs, CNAs, therapists, and family members to deliver coordinated care
• Participate in safety, infection control, and quality improvement initiatives

Qualifications

• Active LPN license (Licensed Practical Nurse) — must be in good standing
• Travel-friendly mindset and availability to work weekends as required
• Current Basic Life Support (BLS) / CPR certification preferred or willing to obtain
• Up-to-date immunizations and ability to complete facility onboarding quickly
• Strong communication skills and a patient-centered approach

Education

• Practical Nursing diploma or certificate (LPN program)
• High school diploma or equivalent

Knowledge & Skills

• Skilled in medication administration and accurate charting
• Knowledge of long-term care practices and skilled nursing routines
• Ability to assess changes in resident condition and escalate appropriately
• Good time management and prioritization during busy shifts
• Team-oriented with strong interpersonal and documentation skills
