Roles & Responsibilities

• Perform thorough speech-language evaluations to establish treatment needs and goals
• Deliver daily treatments tailored to each resident’s plan of care
• Manage patient discharges and ensure clear, timely documentation
• Collaborate with an experienced therapy team and interdisciplinary staff to optimize outcomes
• Follow Continuum Rehabilitation Group (CRG) policies and procedures to keep care consistent and compliant
• Maintain required productivity levels and contribute to a positive facility culture

Qualifications

• 1–3 years of experience (as listed in the job request)
• Prior SNF experience strongly preferred
• Ability and willingness to be on-site to receive guaranteed hours and follow facility policies
• Comfort meeting productivity expectations (90%) and following CRG P&P

Knowledge & Skills

• Skilled in conducting speech-language evaluations
• Experience providing daily therapeutic interventions
• Knowledge of discharge processes and documentation best practices
• Familiarity with Skilled Nursing Facility (SNF) workflows and interdisciplinary collaboration
• Ability to work within productivity frameworks and facility protocols
