Roles & Responsibilities

• Provide patient-centered occupational therapy services in a skilled nursing facility setting
• Assess residents’ functional abilities and develop individualized, meaningful treatment plans
• Deliver skilled interventions that promote independence, safety, and quality of life
• Document progress promptly and accurately according to facility and CRG policies
• Collaborate closely with nursing and interdisciplinary team members to coordinate care
• Maintain professional standards on-site (dress code: black scrubs) and meet facility productivity expectations

Qualifications

• 1-3 years of Occupational Therapy experience (as listed in the job requirements)
• Experience or comfort working in a Skilled Nursing Facility environment
• Ability to be present in the building to receive guaranteed hours and meet onsite expectations
• Willingness to follow Continuum Rehabilitation Group (CRG) policies and productivity targets

Knowledge & Skills

• Skilled knowledge of occupational therapy best practices for SNF populations
• Experience producing clear, timely documentation and assessments
• Ability to work as part of an interdisciplinary therapy team
• Familiarity with productivity expectations and clinical time management
