Roles & Responsibilities

• Provide direct physical therapy interventions under the direction of a licensed physical therapist
• Implement and carry out physician- and PT-directed plans of care for skilled nursing residents
• Document patient progress, treatment response, and outcomes clearly and timely
• Collaborate with nursing and interdisciplinary team members to coordinate patient care
• Educate patients and families on therapeutic activities, safety and home exercise programs
• Adhere to the state Physical Therapy Practice Act and facility policies to maintain high standards of care
• Maintain a safe environment and ensure proper use and upkeep of therapy equipment

Qualifications

• Licensed Physical Therapist Assistant (PTA) or eligible for state PTA licensure
• Graduate of an accredited PTA program (associate degree or equivalent)
• Comfortable working in a Skilled Nursing Facility (SNF) setting
• Any experience level accepted — new grads are welcome
• Strong communication skills and ability to work as part of a team

Education

• Graduate of an accredited Physical Therapist Assistant program (Associate degree typical)

Knowledge & Skills

• Familiarity with common SNF conditions and rehab approaches
• Skilled documentation and clinical charting
• Ability to follow PT-directed treatment plans and physician orders
• Patient-centered approach and strong safety awareness
• Good interpersonal skills for working with residents, families, and staff
