Roles & Responsibilities

• Perform comprehensive home health evaluations and document OASIS assessments accurately
• Develop and teach individualized Home Exercise Programs that promote independence
• Provide skilled physical therapy interventions for orthopedic and neurologic diagnoses
• Chart thoroughly in EPIC and maintain timely, compliant documentation
• Educate patients and families to improve safety, function, and adherence to plans of care
• Manage a productive caseload (may visit up to six patients/day; 30 productivity points standard)
• Coordinate care with nurses, aides, and other disciplines to optimize patient outcomes
• Safely navigate travel between patient homes while demonstrating excellent body mechanics

Qualifications

• Licensed or pending PT license accepted for Oregon; must obtain active license as required
• Minimum 1 year of Skilled Home Health experience required; first-timers not accepted
• BCLS (BLS) certification required
• Experience with OASIS and EPIC charting strongly preferred; clearly note if you lack OASIS experience
• Own reliable transportation and willingness to travel within the service area (locals within ~50-75 miles welcomed)
• Facility requires primary COVID-19 vaccination series plus one booster
• Comfort managing orthopedic and neurologic caseloads including post-op and stroke rehabilitation

• OASIS assessment and documentation
• EPIC electronic charting proficiency
• Skilled assessment techniques and treatment planning
• Home Exercise Program design and patient/family education
• Body mechanics and safe patient handling
• Orthopedics: total hip/knee, shoulder, spinal post-op care
• Neurologic rehab: post-stroke, Parkinson’s, SCI, TBI
• Time management and route planning to maximize patient contact and minimize drive time
