Roles & Responsibilities

• Perform comprehensive home health physical therapy assessments and document findings accurately in EPIC and Oasis
• Develop and implement individualized home exercise programs that promote patient independence
• Provide patient and family education to support safe transitions and ongoing recovery at home
• Manage a typical caseload of up to 6 visits per day while maintaining high quality, patient-centered care
• Coordinate with interdisciplinary team members and community resources to optimize patient outcomes
• Use strong body mechanics and evidence based orthopedic and neurologic rehabilitation techniques in skilled home health settings

Qualifications

• Licensed Physical Therapist in Oregon or pending license accepted (please note pending licenses are accepted)
• Minimum 1 year of skilled home health experience required; first-time travel clinicians are not accepted
• Current BLS/BCLS certification required
• Experience with charting in EPIC and Oasis required; please indicate if you lack Oasis experience
• Primary COVID-19 vaccination series plus one booster required by facility
• Reliable personal transportation for patient visits and ability to travel occasional 50+ mile radii

Knowledge & Skills

• Skilled in assessment, body mechanics, and orthopedics
• Experienced in neurologic rehabilitation including post stroke and movement disorders
• Comfortable with post operative care and total joint rehabilitation
• Proficient at creating and teaching home exercise programs and caregiver education
• Charting experience with EPIC and Oasis
• Strong communication and care coordination abilities in home health settings
