Roles & Responsibilities

• Provide skilled physical therapy evaluation and treatment for residents in a skilled nursing setting
• Develop and implement individualized care and discharge plans that promote function and independence
• Document patient progress and outcomes thoroughly and accurately to meet facility and regulatory standards
• Collaborate with nurses, occupational therapists, speech therapists, and case managers to coordinate care
• Educate residents and families on safe transfers, therapeutic exercises, and home exercise programs
• Perform safe mobility and fall-risk interventions and ensure safe use of assistive devices
• Participate in interdisciplinary team meetings and contribute to clinical problem solving
• Mentor and support rehab assistants and support staff to enhance overall therapy delivery

Qualifications

• DPT or PT credential (DPT preferred); active or eligible for licensure in Indiana
• Clinical skills appropriate for Skilled Nursing Facility practice; new grads welcome
• Comfortable with documentation in electronic medical records and meeting regulatory requirements
• Strong communication and teamwork skills; ability to collaborate with multidisciplinary teams
• Current BLS/CPR certification (or willingness to obtain) and other facility-required trainings

Education

• Doctor of Physical Therapy (DPT) preferred; new graduates considered

Knowledge & Skills

• Knowledge of SNF practice patterns, skilled documentation, and Medicare regulations
• Skilled in therapeutic exercise, mobility training, gait training, transfers, and fall prevention
• Ability to assess functional status and create measurable goals and outcomes
• Proficiency with patient-centered care and family/caregiver education
• Good clinical reasoning, time management, and organizational skills
