Roles & Responsibilities

• Deliver compassionate, patient-centered physical therapy services to skilled nursing residents
• Perform thorough assessments and develop individualized treatment plans that promote function and independence
• Document evaluations, progress notes, and discharge summaries accurately and timely
• Collaborate with nursing, OT, SLP, and care teams to coordinate safe, effective care
• Educate residents and families on home programs, fall prevention, and mobility strategies
• Monitor outcomes and adjust interventions to meet rehabilitation goals and regulatory standards

Qualifications

• PT or DPT (primary skills listed: PT, DPT)
• Open to clinicians at all experience levels — any experience accepted, including New Grads
• Comfort working in a Skilled Nursing Facility (SNF) setting
• Strong documentation skills and ability to collaborate with interdisciplinary teams
• Current credentials and any state-required licensure/certification as applicable for practice in Indiana

Education

• Doctor of Physical Therapy (DPT) or equivalent degree; PT degree holders considered
• New graduates welcome

Knowledge & Skills

• Skilled in patient assessment, mobility training, and restorative care
• Knowledge of SNF rehab protocols, safe transfer techniques, and fall prevention
• Proficient clinical documentation and care-plan development
• Effective communication with residents, families, and clinical teams
• Ability to prioritize caseloads and deliver efficient, high-quality care
