Roles & Responsibilities

• Assess, evaluate, and update therapeutic, rehabilitative, and functional status of patients in a home health setting
• Develop and direct physical therapy treatment plans based on physician orders and patient needs
• Instruct patients and caregivers in the use and care of therapeutic appliances
• Monitor patient reaction to treatment and report changes in condition to the physician
• Supervise home health aides and participate in case conferences to coordinate care
• Prepare clinical and progress summaries, documenting patient goals and outcomes
• Educate families on home exercise programs and safety measures
• Comply with Title 22, Medicare Conditions of Participation, and other regulatory standards

Qualifications

• Minimum 2 years of experience as a Physical Therapist in acute, home care, or rehab setting
• Valid California PT license or compact license eligible
• Current BLS certification from the American Heart Association
• Strong communication, organizational, and documentation skills
• Ability to travel within Yolo County and surrounding areas (Woodland, Davis, Winters, Vacaville, etc.)

Knowledge & Skills

• Proficient in patient evaluation and rehabilitation techniques
• Excellent oral and written communication
• Good organizational and time-management skills
• Ability to work independently and as part of a multidisciplinary team
• Familiarity with home health regulations and documentation standards
