Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, with rotating weekends

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Perform start of care, re-certifications, and evaluations for home health patients
Manage daily visits and ongoing patient care to foster recovery and independence
Develop and implement individualized treatment plans for common diagnoses like CHF, Orthopedic conditions, and Pneumonia
Collaborate with clinical managers and educators for emergency support and guidance
Document patient progress accurately and timely using provided technology

Qualifications

Current Physical Therapist licensure in Kentucky
BLS certification required
Home health experience preferred, but new grads with strong clinical rotations welcome
Reliable transportation and willingness to travel to patient homes within assigned coverage area

Knowledge & Skills

Strong clinical assessment and treatment planning skills
Excellent communication and patient education abilities
Ability to work independently and manage a caseload efficiently
Comfortable using electronic health records and provided tablet/phone
