Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Perform start of care, recertifications, and resumption of care visits for home health patients
Manage a daily caseload of 6-7 points, including regular visits and admissions
Collaborate with clinical coordinators and sister agencies for support
Conduct safety evaluations and occasional hospice teaching visits
Document patient progress and coordinate care with interdisciplinary team

Qualifications

Active North Dakota Physical Therapy license required
Current BLS certification required
Valid driver's license and auto insurance are a must
Previous home health experience and OASIS training strongly preferred

Knowledge & Skills

Home health and hospice care principles
OASIS documentation and assessment skills
Excellent time management and independent work ability
Strong communication with patients, families, and healthcare teams
