Roles & Responsibilities

Provide physical therapy treatments under the supervision of a licensed Physical Therapist to help patients regain mobility
Assist patients with mobility exercises, strength training, and individualized rehabilitation programs
Implement treatment plans designed by the Physical Therapist and adapt approaches as patients progress
Monitor patient progress, accurately document treatment outcomes, and communicate changes to the care team
Educate patients and families on therapeutic exercises and safe mobility techniques to support independence
Maintain compliance with facility policies, healthcare regulations, and best practices for patient safety

Qualifications

Active California PTA license (required)
Current BLS/CPR certification (required)
Experience in a Skilled Nursing Facility (SNF) preferred
Strong communication skills and the ability to work effectively as part of a multidisciplinary team

Knowledge & Skills

Physical therapy treatments and bedside patient care
Mobility exercises and strength training techniques
Design and implementation of rehabilitation programs under PT supervision
Clinical documentation and monitoring of patient progress
Patient and family education on therapeutic and safety practices
Familiarity with SNF workflows and regulatory compliance
