Roles & Responsibilities

• Evaluate residents' functional mobility and design individualized treatment plans
• Implement evidence-based therapeutic interventions to improve strength, balance, and independence
• Document clinical progress clearly and timely, ensuring accurate therapy records
• Collaborate with nursing, social work, and interdisciplinary team members to coordinate care
• Educate residents and families on safe mobility strategies and home exercise programs
• Supervise and mentor therapy assistants and support staff when applicable
• Contribute to discharge planning and transitions of care to support successful outcomes
• Maintain compliance with facility policies and regulatory requirements

Qualifications

• Current, unrestricted Physical Therapist license (ability to obtain NH license if relocating)
• BLS/CPR certification (or willingness to obtain upon hire)
• Experience working in a Skilled Nursing Facility or post-acute/rehab setting preferred
• Strong clinical reasoning, assessment, and documentation skills
• Excellent communication and teamwork mindset — you collaborate well across disciplines

Education

• Graduation from an accredited Physical Therapy program (DPT preferred)

Knowledge & Skills

• Skilled in therapeutic exercise, gait training, balance training, and mobility assessment
• Familiarity with ADL training, assistive devices, and durable medical equipment
• Comfortable with electronic documentation and clinical outcome measures
• Understanding of Medicare, rehab documentation standards, and regulatory requirements
• Effective patient and family education skills and motivational coaching
