Roles & Responsibilities

• Provide skilled physical therapy evaluations and develop individualized treatment plans that help patients regain function
• Deliver hands-on treatment, therapeutic exercise, gait training and use of modalities to support recovery
• Document patient progress thoroughly and communicate outcomes with the care team and family members
• Collaborate with nurses, occupational therapists, speech therapists and physicians to coordinate patient-centered care
• Discharge planning and patient education to ensure safe transitions and ongoing recovery at home
• Maintain compliance with facility policies, safety protocols and regulatory standards

Qualifications

• Current or compact-eligible Physical Therapist license (PA license preferred or ability to obtain prior to start)
• Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
• Current BLS/CPR certification
• Experience in inpatient, subacute or skilled nursing/rehab settings preferred
• Strong clinical assessment skills, excellent documentation, and effective communication with patients and care teams
• Ability to pass required background checks and credentialing for the facility

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor’s in Physical Therapy or equivalent considered

Knowledge & Skills

• Skilled in musculoskeletal and neuromuscular assessment and intervention
• Proficiency with therapeutic exercise, manual therapy and functional mobility training
• Experience with safe transfers, gait training and assistive device training
• Comfortable using electronic health records (EHR) for documentation
• Familiarity with outcome measures and goal-driven treatment planning
• Collaborative mindset and strong interpersonal skills to work within an interdisciplinary team
