Roles & Responsibilities

• Provide compassionate, hands-on nursing care to patients in a rehab/uptown health setting
• Administer medications and treatments accurately and safely
• Perform ongoing patient assessments and communicate changes promptly
• Document patient care and progress clearly in the electronic health record
• Collaborate with nurses, therapists and providers to support patient-centered plans
• Assist with patient mobility, ADLs and rehabilitation goals to maximize outcomes
• Support discharge planning and caregiver education to ensure smooth transitions

Qualifications

• Current LPN license in New Mexico (or eligible to obtain state licensure)
• Active Basic Life Support (BLS) certification or ability to obtain upon hire
• Comfort working 12+ hour day shifts as needed
• Strong communication skills and ability to collaborate on a multidisciplinary team
• Current immunizations and ability to meet facility onboarding requirements

Education

• Vocational nursing diploma or equivalent (LPN program completion)
• High school diploma or GED required

Knowledge & Skills

• Solid knowledge of medication administration and documentation best practices
• Skilled in patient assessment, vital signs and wound/productive care basics
• Familiarity with rehabilitation care practices and assisting with ADLs
• Comfortable using electronic health record systems and charting accurately
• Ability to prioritize, multitask and respond calmly in fast-paced situations
