Roles & Responsibilities

• Provide compassionate bedside care and support to patients in a rehabilitation-focused area
• Administer medications and treatments accurately and safely according to policy
• Monitor and document patient condition, vital signs, and clinical changes
• Collaborate with the interdisciplinary team to create and implement care plans
• Assist with rehabilitation activities and support patient mobility and recovery goals
• Educate patients and families on care needs, safety, and discharge planning
• Maintain accurate, timely documentation in the chart and communicate handoffs clearly

Qualifications

• Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) license — LPN encouraged to apply
• Active license to practice in New Mexico required
• Current Basic Life Support (BLS) / CPR certification
• Comfortable working 12+ hour night shifts (6:00 PM - 6:30 AM)
• Experience in rehabilitation or post-acute care preferred
• Ability to pass background check, drug screen, and required immunizations

Education

• Graduate of an accredited LPN program (for LPN) or Associate/Bachelor of Science in Nursing (for RN)
• Additional certifications in rehab care or specialty courses are a plus

Knowledge & Skills

• Strong patient assessment and observation skills
• Medication administration and adherence to safety protocols
• Experience supporting rehab interventions and mobility assistance
• Clear, compassionate communication with patients, families, and team members
• Proficient documentation and time management in a busy clinical environment
• Ability to prioritize tasks and escalate changes in condition appropriately
