Roles & Responsibilities

• Provide skilled physical therapy evaluation and treatment in the home health setting to improve patient function and safety
• Develop individualized care plans and measurable goals based on patient needs and best practices
• Deliver evidence-based therapeutic interventions such as gait training, balance work, and therapeutic exercise
• Document timely, accurate, and complete clinical notes in the electronic health record (Epic preferred)
• Collaborate with interdisciplinary team members and families to coordinate care and optimize outcomes
• Perform objective assessments, track progress, and adjust treatment plans to meet patient goals
• Educate patients and caregivers on safe mobility, fall prevention, and home exercise programs

Qualifications

• Current Physical Therapist license (state licensure verification required)
• BLS certification (required)
• Home health experience preferred; be comfortable providing autonomous, in-home care
• Epic experience preferred but not required
• Successful completion of Trinity specialty competency testing and skills checklist as part of onboarding
• Up-to-date immunizations and required health screenings (MMR, Hep B, Tdap, COVID-19, influenza, PPD/IGRA, Varicella, drug screen, fit test, etc.)
• Ability to pass background checks and licensure/certificate verification

Knowledge & Skills

• Strong knowledge of home health clinical practice and safety in the home environment
• Proficient documentation skills and familiarity with electronic health records (Epic preferred)
• Excellent clinical reasoning for assessment, treatment planning, and discharge planning
• Skilled in therapeutic exercise, mobility training, balance/fall prevention, and patient/caregiver education
• Clear communication and collaboration skills for working with patients, families, and interdisciplinary teams
• Time management and caseload prioritization for an efficient home health practice
