Roles & Responsibilities

• Provide compassionate, patient-centered bedside care and support
• Administer medications and treatments safely and accurately
• Perform ongoing patient assessments and monitor vital signs
• Document care thoroughly and maintain accurate, timely records
• Collaborate with nurses, providers, and interdisciplinary team members to coordinate care
• Assist with wound care, ADLs, and comfort measures to promote healing and dignity
• Support discharge planning and patient education to improve outcomes
• Adhere to infection control, safety protocols, and facility policies at all times

Qualifications

• Active Licensed Practical Nurse (LPN) credential in the state of practice
• Local candidates only (commutable to Rochester, NH) as specified
• Availability for Shift 2 (3:00 PM - 11:30 PM) and Every Other Weekend (EOW)
• Current BLS/CPR certification or willingness to obtain prior to start
• Professional, team-oriented attitude with strong communication skills
• Previous LPN experience in long-term care or similar settings is a plus

Education

• Practical Nursing diploma or equivalent completion of an accredited LPN program

Knowledge & Skills

• Solid medication administration and documentation skills
• Patient assessment and monitoring, including vital signs
• Familiarity with long-term care routines and resident-centered care
• Ability to prioritize tasks in a fast-paced clinical environment
• Comfortable using electronic health records and standard charting
• Strong teamwork, situational awareness, and patient advocacy
