Roles & Responsibilities
- Provide skilled physical therapy evaluation and treatment to patients in their homes, improving function and independence
- Develop clear, personalized treatment plans and update goals based on patient progress
- Document all patient care accurately and timely in HCHB to ensure continuity and compliance
- Coordinate care with nurses, therapists, and caregivers to deliver integrated home health services
- Educate patients and families on safe mobility, fall prevention, and home exercise programs
- Manage your patient schedule and travel between visits efficiently while maintaining high-quality care
- Advocate for patient needs and communicate changes in condition promptly to the interdisciplinary team
Qualifications
- Current, active Physical Therapist license in North Carolina (required)
- Minimum 2 years of experience, with home health experience strongly preferred
- Experience with home health and hospice settings preferred for nursing/therapy crossover
- Comfortable performing patient visits in private homes; reliable vehicle required
- Basic Life Support (BLS) certification or willingness to obtain upon hire
- Strong communication skills, compassion, and ability to work independently
Education
- Doctor of Physical Therapy (DPT) or Master's in Physical Therapy (entry-level PT degree required)
Knowledge & Skills
- Proficiency with HCHB documentation system or similar home health EMR
- Thorough knowledge of home health assessment and treatment planning
- Skilled in patient and caregiver education, safety assessment, and functional mobility training
- Excellent time management and route planning for efficient patient visits
- Ability to collaborate effectively with interdisciplinary teams and community resources
