Contract Type: Contract

Shift Type: Shift varies: 7:30 AM - 4:00 PM or 8:00 AM - 4:30 PM, includes rotating weekends and holidays

Hours per Week: 40

Candidate Pay Rate: $71 per hour

Roles & Responsibilities

Develop daily patient lists and review patient charts to assess needs
Facilitate goals of care discussions and educate patients on disease processes
Assist with Advance Directives and POLST documentation
Conduct family meetings and provide end-of-life discussions and support
Attend daily rounds with the palliative care team
Collaborate with a multidisciplinary team including physicians, NPs, and RNs

Qualifications

Must have 2-5 years of recent experience in palliative care and/or hospice setting
BLS certification required
ELNEC certification preferred
Strong acute care experience and clinical thinking skills
Excellent communication and organizational skills
Flexibility and a desire to serve patients and families
Must not have been employed by any CommonSpirit facility (including CHI and Dignity) within the past year

Knowledge & Skills

Expertise in palliative and hospice care principles
Acute care nursing skills
Proficiency with Cerner EMR
Advanced clinical thinking and problem-solving
Effective communication and interpersonal skills
Strong organizational and time management abilities
Flexibility to adapt to changing patient needs
