Location: Santa Maria, California

Start Date: 06-15-2026

End Date: 09-14-2026

Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, with every 3rd weekend

Hours per Week: 40

Candidate Pay Rate: $64 per hour

Roles & Responsibilities

Provide skilled physical therapy services to patients in their homes, helping them regain independence
Develop and implement individualized treatment plans based on patient goals and needs
Conduct comprehensive assessments including Oasis documentation
Collaborate with interdisciplinary team members to ensure coordinated care
Educate patients and families on home exercise programs and safety

Qualifications

Current California Physical Therapist license
BLS certification required
Minimum of one year experience as a Physical Therapist
Experience with Oasis documentation required
Home health experience preferred but not required
Proficiency with Cerner EMR preferred

Knowledge & Skills

Strong clinical skills in orthopedics, neurology, and general rehabilitation
Excellent communication and interpersonal skills
Ability to work independently and manage a caseload
Knowledge of home health regulations and Oasis guidelines
Time management and organizational skills
