Roles & Responsibilities

• Conduct comprehensive physical therapy assessments and develop individualized treatment plans
• Implement therapeutic interventions to improve patient mobility, strength, and function
• Collaborate with an interdisciplinary team to ensure coordinated patient care
• Educate patients and their families on treatment goals, exercises, and home management
• Maintain accurate and timely documentation of patient progress and outcomes
• Adapt treatment techniques to meet the unique needs of each patient in a post-acute setting

Qualifications

• Active California Physical Therapist license required
• Graduate of an accredited physical therapy program
• BLS certification from the American Heart Association
• Minimum of 1 year of clinical experience preferred
• Strong interpersonal and communication skills
• Ability to work independently and as part of a collaborative team

Knowledge & Skills

• Expertise in evaluation and treatment of orthopedic, neurological, and geriatric conditions
• Proficiency with electronic medical records systems
• Knowledge of evidence-based practice and rehabilitation protocols
• Time management and organizational skills to handle a dynamic caseload
• Compassionate and patient-centered approach to care
