Licensed Practical Nurse (LPN) - Long Term Care

Location: Breckenridge, MN

Start Date: 07-06-2026

End Date: 10-05-2026

Contract Type: Travel Contract

Shift Type: Cycle scheduling includes 6:00 AM - 2:30 PM, 2:00 PM - 10:30 PM, and 10:00 PM - 6:30 AM; requires working every other weekend and holiday.

Hours per Week: 36

Candidate Pay Rate: $46 per hour

Roles & Responsibilities

Provide direct patient care to residents in a long-term care facility
Administer medications and treatments as prescribed
Monitor patient conditions and report changes to the healthcare team
Collaborate with CNAs and other staff to ensure quality care
Document patient care accurately using PointClickCare EMR
Maintain a safe and comfortable environment for residents

Qualifications

Current and valid LPN license in the state of Minnesota
BLS certification required
Minimum of 1 year of LPN experience preferred
Strong teamwork and communication skills
Ability to work rotating shifts including weekends and holidays

Knowledge & Skills

Knowledge of long-term care regulations and best practices
Skills in medication administration and monitoring
Compassionate patient care approach
Proficiency with electronic medical records (PointClickCare experience a plus)
Ability to work in a fast-paced environment
