Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Conduct comprehensive patient assessments and develop individualized care plans
Provide skilled physical therapy services in patients' homes
Perform start-of-care visits for orthopedic patients
Collaborate with interdisciplinary team to ensure coordinated care
Document patient progress accurately in HCHB electronic health record
Educate patients and families on therapeutic exercises and safety

Qualifications

Current Physical Therapist license in California
BLS certification required
Home Health experience preferred; travelers with less than 2 years considered if they have home health background
Experience with HCHB (HomeCare HomeBase) EMR is a plus

Knowledge & Skills

Orthopedic rehabilitation including joint replacements
Geriatric care and management of comorbidities (CHF, COPD)
Patient assessment and care planning
Effective communication and patient education
