Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Conduct thorough Oasis assessments and start-of-care visits for home health patients
Develop and implement individualized physical therapy plans to restore mobility and function
Educate patients and families on fall prevention, therapeutic exercises, and home safety
Collaborate with interdisciplinary team to ensure comprehensive patient care
Document patient progress accurately using tablet-based system

Qualifications

Active Physical Therapist license in Indiana (or compact eligible)
Home health experience preferred, especially with Oasis documentation
Ability to manage 4-6 patient visits per day using point system
Valid driver's license and reliable transportation for travel across four counties

Education

Doctorate or Master's degree in Physical Therapy from an accredited program

Knowledge & Skills

Strong knowledge of Oasis assessments and start-of-care protocols
Excellent time management and organizational skills
Ability to work independently in a rural home health setting
Compassionate communication with elderly patients and families
