Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide physical therapy services to patients in their homes
Develop and implement individualized treatment plans
Assist patients recovering from surgeries, falls, and other conditions
Collaborate with interdisciplinary team for continuity of care
Document patient progress in HomeCare/HomeBase EMR

Qualifications

Current North Dakota Physical Therapy license required
BLS certification required
Minimum 1 year of home health experience
Valid driver's license and auto insurance required
Ability to travel within a 60-mile radius of Valley City

Knowledge & Skills

Home health experience
Proficient with HomeCare/HomeBase EMR
Knowledge of common home health diagnoses
Excellent communication and interpersonal skills
Compassionate patient-centered approach
