Roles & Responsibilities

• Conduct start of care assessments, recertifications, and resumption of care visits for home health patients
• Develop and implement individualized treatment plans to improve patient mobility and function
• Perform safety evaluations and provide patient and family education
• Collaborate with clinical coordinators and directors for optimal patient outcomes
• Manage a daily caseload of approximately 6.5 points, including regular visits and admissions
• Document patient progress accurately and timely in accordance with OASIS guidelines

Qualifications

• Current BLS certification (American Heart Association)
• Active North Dakota Physical Therapy License
• Valid Driver's License and current Auto Insurance
• Home Health experience preferred
• OASIS training and experience preferred
• Willing to work a short orientation period (1-2 weeks) with a preceptor
• First-time travelers welcome to apply!
