Location: Woodland, CA

Start Date: 07-07-2026

End Date: 10-06-2026

Contract Type: Contract

Shift Type: Monday-Friday, 8:30 AM - 5:00 PM, with occasional Saturday rotation once per month

Hours per Week: 40

Candidate Pay Rate: $58 per hour

Roles & Responsibilities

Assess, evaluate, and update therapeutic/rehabilitative/functional status of patients
Develop and direct physical therapy treatment plans based on physician's plan of care
Instruct patients and families in use of therapeutic appliances and home exercise programs
Monitor and report patient progress and changes in condition to the physician
Supervise home health aides and homemakers involved in patient's rehabilitation
Prepare clinical and progress summaries and maintain accurate patient records
Participate in case conferences and peer consultation
Comply with Title 22, Medicare, Medi-Cal, OSHA regulations and licensure requirements

Qualifications

Minimum 2 years of experience as a Physical Therapist in an acute, home care, or rehab setting
Current BLS certification required
Excellent oral and written communication skills
Strong organizational skills and ability to manage a caseload
